1. A 25-bed facility was struggling to maintain operational processes and reach its target patients. SCP’s patient engagement team helped educate local employers on employee healthcare needs, provided necessary reporting to find missed referral opportunities, and more.
Within two years, the hospital had 34 new employer relationships with 8.690 covered lives, a 71% increase in general/preventative health visits, and a total ROI of over $288K.
2. A 50-bed facility was facing challenges in achieving their target patient volume, and providing effective navigation to their patients. SCP built relationships with local employers and provider groups; contracted with local payors; launched an ED and urgent care patient navigation program; and established a physician referral line to connect patients to the right provider.
In the first 18-24 months, the facility gained 600+ new patient referrals through the ED navigation program, saw a 137% increase in general/preventative health visits, and achieved an ROI of $777K.
3. A 99-bed facility wanted to improve their patient care coordination abilities, and reach a larger percentage of the commercially insured population. SCP supported those goals by deploying corporate health consultants to visit local employers, build rapport, and organize events to address employee needs. SCP also put service coordinators in place to provide patient navigation to the area population.
In just one year, the facility gained 122 new patient referrals through health events, generated over $1M in new business, and created coordination efficiencies in over 1,700 patient appointments.
